REGISTRATION FORMS
Students must pre-register by:
~ Completing the Application for Admission Form below and providing a *non-refundable $60 registration fee to the school (checks only).
Already enrolled at another preschool program for fall 2010? Provide a receipt or cancelled check and we will deduct that amount from our $60 registration fee for fall 2010 enrollment.
F O R M S:
1. Download Application for Admission Now (Used to enroll) 2010 - 2011 school year
2. Download Enrollment Form Now (Due before 1st Day of Class) 2010 - 2011 school year
3. Download Emergency Contact Form Now (Due before 1st day of Class)
4. Download Child Health Assessment Now (All physical forms expire every 12 months, current form due within 50 days of child's first day at KM)
5. Contract (filled out by the teacher and signed by both the parent and teacher)
